こんにちは、Team WADA学生インターンのチェコマサリク大学医学部6年の飯嶋です。

ハワイ大医学部生と合同企画、Clinical Dojo with JABSOMについてのレポートです。

今回で第６回を迎えたこの臨床推論会、この企画の大ファンである私は、５回目の参加でした。時差の関係で朝6時に寝惚け眼でZOOMにログイン、すると前回を遥かに凌駕する参加人数の多さに驚き一瞬で目が覚めました。

今回の参加者はオブザーバーも含め、なんと２７名。日本、ハワイのみならず、米本土、上海、ヨーロッパからも参加とまさにグローバルそのもの。参加者が数名だったプログラム初期に比べ、かなり成長しました。そしてクオリティも回を重ねるごとに向上しています。最初は役割分担、準備、話の進め方など全くわかりませんでしたが、何回か経験するうちに徐々にPBL スタイルに慣れてきたように感じます。PBLのプロであるJABSOM 生徒達の協力、手ほどきに心から感謝です。。

今日の症例は、ハワイ大2年生からの提示でした。

患者は6歳アフリカ系アメリカ人男児、主訴は2日間の肉眼的血尿

皆さんは何を考えますか？？

ＰＢＬのケースディスカッションでは、上記の様に参加者に情報が与えられ、それをもとに議論を展開していきます。司会のガイドのもと、参加者がアクティブに意見を出し合い、知識を共有、鑑別診断をあげていきます。子供に多いPSGN、IgA vasculitis/nephropathy、人種を考慮したsickle cell diseaseや、 trauma, UTI, porphyria, lead poisoning, ARPKD, ADPKD, Wilm’s tumorなど多岐にわたる鑑別があがりました。最後に答え合わせがありますが、正しい診断をあてるのが目的ではなく、あくまで与えられた情報をもとにどう考察していくかに重きを置き、ディスカッションが行われます。

私が思うこのプログラムの魅力は何といってもハワイ大の学生との交流です。とにかく人柄が良く、協力的です。パンデミックで主流となったオンラインセミナーでは、場所の制約や移動の手間がなくなり便利になった反面、参加者がアクティブに発言し、議論に参加するという面ではすこし敷居が高くなったように感じます。ましてや、英語での発言となるとさらにアクティブ度が下がるのは必然。そんなわれわれ日本人のお悩みを知ってか知らずか、ハワイ大の学生はものすごく協力的に発言を促してくれます。私が参加した最初の回で、仕切り役だった学生が放った最初の言葉が “Don’t be afraid of making mistakes”。そしてそれを助長するように、我々の言葉足らずを補足し、一人ひとりの発言をしっかり拾いディスカッションに落とし込むなど、とにかく発言しやすい環境作りをしてくれます。

ハワイ大医学生のこのアロハ精神に感謝するとともに、自分たちが逆の立場で外国人に教えてたりする場合はこうすると相手が助かるのかなど、とにかく学びや気づきが多いです。

Hello, I am Akimasa Iijima, a final year medical student at a medical university in the Czech Republic.

It is my great pleasure to write a report and personal feedback on our 6th PBL session with JABSOM students. It was my 5th participation (sucks I missed the very first one), and I was glad to see the program grown so big! Today, we got a total of 27 participants from all over the world! That means it must have been a real challenge for the organizer to pick a time that works for everyone in FIVE different time zones! Not just the size, but the quality of the session is also getting better each time as we are becoming more familiar with the PBL. Thanks to the professionals at JABSOM, who provide us hands-on experiences on how to run the PBL session.

Today’s case was prepared by MS2 at JABSOM. 

The patient was a 6-year-old African American boy with 2-day history of gross hematuria.

We discussed the case based on the given information under the modulator’s guide and tried to come up with questions to ask the patient, potential diagnoses, and the likelihood of each DDx. PBL is very organized and well structured, and it involves a lot of active thinking. It also provides us great opportunities to explain knowledge to others in an understandable manner.

For today’s case, we came up with DDx of IgA vasculitis/nephropathy, sickle cell disease, trauma, UTI, porphyria, lead poisoning, ARPKD, ADPKD and Wilm’stumor etc. Thanks to many active participants, we had very enjoyable discussions!

One of the reasons I am a big fan of this program is that we get to interact with students from JABSOM who are very supportive and respectful!! JAMSOM students are extremely good at making a “safe environment” for ESLs to talk in English by being attentive, helping us to come up with the word, and facilitating to speak out. “Don’t be afraid of making mistakes.” I still vividly recall it was the very first thing that one of the JABSOM students said to encourage us in the early session. The biggest challenge we have in having an online session is making participants active. As we all have experienced ourselves, people tend to be less active when speaking into the computer screening with pajamas on than being in the in-person classroom settings. It is even more difficult when there is a language barrier. Another big thanks to JABSOM students for allowing us to actively learn in a friendly and safe environment where all participants are treated like Ohana.Maybe it is a time for us learn some Aloha spirit as well!
