師走の忙しい中、みなさんはどんな予定を年末にたてていますか？

私は、そろそろ本格的に臨床留学に向けてobservershipを受け入れてくれるところを探したいなと

思っています。

さて、今日は毎週土曜日に開催していただいているNs.Rossi先生により医療面接の勉強会の模様をお伝えしようと思います。

今回の患者さんは働き盛りの55才男性。激しい腹痛と吐き気が数週前から継続をしている方でした。

黒色便も自覚をしており、不眠症も合併をしているため抗うつ薬を家庭医に処方されたとのことでした。

今回、Rossi先生からいただいたお題は二つ！

・患者さんにとって適切な検査を行うこと。またそれがなぜ必要なのかを説明すること。

・現在の患者さんの状況を説明し、今後どのように治療を薦めていくかを説明すること

私は当初、黒色便がみられることから消化管の出血を疑い、上部消化管内視鏡検査の実施を考えていました。しかし、その前にラボデータとして貧血があるかを確認したほうがいいのでは・・などとも考えていました。

特に忙しく、かつ、不眠症を患うくらいに心配ごとを抱えている患者に対して、あまり刺激をすることなく現状を伝えるのはどのようにしたらいいのか。

また、米国へ出張が控えている患者に対して、絶対に今検査をしなければならないと伝える根拠はなにか、などと考えました。

このような問題は正解がないかと思いますが、できるだけEvidence Based で、かつ一番患者さんの身体にとって安全なことを選んでいきたいと思います。

このように、Ns.Rossi先生の勉強会では単なる英語の向上を目指すのではなく、臨床現場でどのように考えたらいいかもプロの目線で教えていただけます。

Hello everyone! I’m Kanako Fukuda, a fourth-year student at Yamaguchi University School of Medicine!

So, starting off from the random question, “what are your plans for the end of the year in this busy season?”

I’m thinking of looking for a place that accepts observerships for clinical study abroad in earnest.

So, today  I’d like to report on the medical interview workshop held by Dr. Rossi every Saturday.

This time, the patient was a 55-year-old man with busy schedule.  He had been experiencing severe abdominal pain and nausea for several weeks.

He was also aware of black stools and was prescribed antidepressants by his family doctor for insomnia.

The questions I was to asked was below

1) Start a patient examination and explain appropriate treatment

2) Explain to this patient about his current condition and your treatment

Initially, I suspected bleeding in the gastrointestinal tract due to the black stools , so my option was to perfrom an upper gastrointestinal endoscopy. However, I also thought that it would be better to check for anemia as lab data before doing so.

I wondered how I could communicate the current situation to a particularly busy and anxious patient who was suffering from insomnia, without provoking him too much.

I also wondered how I could tell a patient who was about to go on a business trip to the U.S. , but at the same time he absolutely had to have an examination now.

I think there is no right answer to these questions, but I would like to choose the safest thing for the patient’s body based on evidence as much as possible.

As you can see, Dr. Rossi’s study group does not just aim to improve your English, but also teaches you how to think in a clinical setting from a professional perspective.
