第8回 Clinical Dojo with JABSOM

平素よりお世話になっております、学生インターン副代表の菱沼俊哉と申します。11月21日日曜日15時〜17時にて、第8回Clinical Dojoを無事開催できましたので、この場をお借りしてご報告いたします。今回を含めて残すところ5回となりました本プログラムですが、多くの方のサポートのもとここまで大きく成長できていることに心より感謝申し上げます。今回から新たにBreakout rooms機能を用いて、新たにご参加いただいた方が発言しやすい雰囲気づくりを心がけました。運営中、至らない点も多々あったことと思いますが、ご参加いただきました方々にこの場をお借りして御礼申し上げます。

そして、今回からハワイ大学医学部（JABSOM—John A. Burns School of Medicine）の学生にもレポートを寄稿していただくことになりました。JABSOM MS1のDillon Putzlerより、まずはこちらをご一読ください！

My name is Dillon Putzler, I’m a first-year student at the John A. Burns School of Medicine. On November 20^th, 2021, we had our eighth Clinical DOJO, hosted by Team WADA! Clinical Dojo gathers students from across the globe to practice speaking English in a medical setting, and to deepen our understanding of disease by collaboration with our medical peers. The goal of each Clinical dojo meeting is to work through multiple pages of a Problem-based learning (PBL) case. These cases are carefully constructed to include important key clinical findings and how we can relate those findings to various medical conditions. While the PBL case highlights learning issues for the meeting, I always learn the most during discussion with my peers! In PBL cases, we are not given the diagnosis until we work our way through each page by highlighting the key facts, problems, hypotheses, and details we would like to know for the next page. This structure leads to a natural, and educational, discussion about various findings while improving our clinical reasoning skills at the same time. The goal of PBL is not to predict the “correct” disease. Instead, the goal of PBL is to deepen our understanding of how a disease causes the symptoms that it does, and how it can be compared and contrasted to other diseases.

In the most recent meeting over the weekend, 23 students joined the session! In the past, we had all of the students in one large Zoom room, but the program has been growing so much we decided to split the group into smaller rooms so we could all feel more comfortable in both getting to know each other and discussing the case. This case was a difficult one, a young woman presented with a three-day history of knee pain that ended up being Septic arthritis from a N. Gonorreae infection! I’m near start of my medical education, and even though I knew almost nothing about the many causes of knee pain or the symptoms of a N. Gonorreae Infection, I was able to participate in the conversation and learn from my peers in classes above me. Team WADA is a wonderful place to learn how to discuss complex clinical cases with other medical professionals. I would like to highlight a quote from our post session review that emphasizes how friendly, and comfortable, the environment is at the Clinical Dojo:

“Actually, this was the first time participating (I was an observer last time) and I had lots of fun as well as learning. I was shy at the beginning, but other participants were very nice, and I felt comfortable in the end. I would like to participate again.”

I was surprised to learn how few resources there are in other countries to practice English in a medical setting. Clinical DOJOs offered by Team WADA provides a space for students to gain proficiency and confidence in English. Native English speakers are also given a space to see the difference in medical training across the globe, and to learn cultural difference in the physical exam process. We also get to help international students get closer to their goal of practicing in the US!

If you have some time, stop by at one of our meetings! It’s a wonderful environment for learning and growth. I am truly fortunate to have met such kind and compassionate people this early in my medical education and look forward to seeing the group grow!

Clinical Dojoを立ち上げた経緯、実際のプログラムにおける他にはない特徴などなど、全てがここに詰まっています！I deeply appreciate it, Dillon!

さて、今回はハワイ大学の学生からの症例提示でした。24歳女性の3日間続く膝の痛みをメインテーマに、次の1ページ目からセッションを開始いたしました。皆さんはどういった鑑別疾患を考えますか？

今回は、3つのBreakout roomsに分かれて少人数でのグループディスカッションを行いました。当日使用したシートより、一部を抜粋して掲載致します。

今回、想定していた診断はN. gonorrhea（淋菌）による化膿性関節炎septic arthritisでした。最後のTake home messageセッションでは女性患者の診察にあたってSTDや妊娠を念頭に問診を行うことの重要性などが挙がりました。疾患自体を当てることのみならず、それに至るまでの過程から多くのことを学べるのが、このJABSOMスタイルの素敵なところです。

英語の勉強について、USMLEをとってからやるべき、等々色々な意見があることは承知しております。この勉強会を立ち上げることの意義を考えたこともありました。しかし、やはり医療の根本にあるのは言語でありコミュニケーションであるはずです。アメリカに行ってしまえばそれは英語学習にとって最高の環境であることに間違いはないですが、学生のうちに日本でできる範囲の中で最大限の努力をすることは決して無駄な経験ではないと強く信じています。そして、日本とハワイをはじめとする沢山の学生の多大なる協力のもと、今までにない新しい英語学習の場を提供できていることを大変誇りに思っております。Evidence-basedであるこちらのショートクリップもぜひご覧ください。

以前より繰り返しになりますが、この企画は決して成績優秀な学生をターゲットとして作られたものではありません。むしろ、海外に興味があるけど英語の勉強の仕方がいまいちよくわからない、そもそも英語を使う場所があまりなくて困っているなど、そういった環境にいる学生に来てもらえたらとても嬉しいです。かくいう自分もアメリカにはまだ行ったことがありません。帰国子女であろうとなかろうと関係ありません。「自分は帰国子女ではないから…。」と言い訳するのが嫌だったこともこの企画をはじめた動機の一つです。それでも、実際にここまで企画・実行できているのですから、臆することなく気軽に参加してもらいたいです！Team WADA学生インターンとハワイ大学の素敵な学生たちと共に、皆さんのご参加をお待ちしております！
