役職：Advanced fellowship position in LVAD, Heart Transplant & Minimally Invasive Surgery
期間：1 year (commencing July 1, 2019(2020の間違い？). 2nd year reappointment option, dependent upon satisfactory progress in training, performance of all duties and continued visa clearance, if applicable.)
必要な書類①：Curriculum Vitae

必要な書類②：Three letters of support from previous training programs or current employer. All letters should be addressed to Yoshifumi Naka, Director of the Cardiac Transplantation and Mechanical Circulatory Support Programs. Letters should be sent via email.

必要な書類③：Hard copies of USMLE scores I, II and III

必要な書類④：Copy of ECFMG certificate

必要な書類⑤：Copy of board certification from country of Cardiac Training

必要な書類⑥：Eligibility for H1 or O1 visa, or Green Card

必要な書類⑦：Current FCVS profile preferred. Selected applicant must use FCVS for NYS Licensure process
