Team WADA学生インターンに所属している旭川医科大学６年の橋本侑樹です。
ハワイ大学学生とTeam WADA学生インターンで、臨床推論勉強会を行う企画であるClinical DOJO (Team WADA-JABSOM PBL Program)、第5回にあたる今回はJABSOMから7名の学生が参加してくださり、白熱した議論となりました！
また、今回はQuizizzを使った確認テストなど新たな要素も導入され、とても充実したPBLになったと思います。それでは様子を紹介させていただきます！
今回はJABSOMからの新メンバーを含めて15人でPBLをスタートしました。
初対面ということで皆さん緊張した様子でしたが、一人ずつ自己紹介を行って一気に緊張が解けていきました。
症例提示は本企画のディレクターである菱沼さんによって行われました。若年男性の呼吸困難をテーマに、想定される疾患や聞くべき患者さんの情報として何が考えられるか意見を言い合いました。
たくさんの学生が積極的に発言されていて、とてもよい雰囲気の中で議論を交わすことができました。
症例提示が終わり、Quizizzを使用した確認テストを行いました！
テストでは本症例から学ぶことのできる重要なポイントを確認することができました。また、我らがTeam WADAに関する問題やハワイに関する問題などを通して、JABSOMの学生さんとより親しくなれたと感じます！
菱沼さんのスムーズな司会進行、学生が積極的に思考できるよう工夫の凝らされた症例提示やクイズのおかげで、楽しみながらディスカッションを行うことができました！
私は初回から一回を除いてPBLに参加させて頂いています。その中でとりわけ僕がこのPBLに参加させていただいてよかったと思う点を2つ紹介します。（もっとありますが、今回は絞って）。
１つは他の大学、他の国の学生との交流を通して文化の違いを学ぶことができる点です。英語でのディスカッションはもちろん刺激的ですが、医療に関わる文化の違いを知ることができるという点も魅力的です。国が異なると医療を取り巻く環境や文化も異なります。そんな異なる環境で医学を学ぶ学生と交流する事は、新たな気付きを与えてくれます。今回では医療コストの考えからCTは安易に撮ることができないことや、アメリカでは医療費が日本よりも高いため健康診断をルーティンで受ける人は殆どいないということを学びました。医学について英語で学びながら、異文化との交流も行うことができるという点は素晴らしいと思います。
２つ目は臨床推論における問診の重要性を知ることができるという点です。多くの人が意見を出し合うと、問診だけでこんなに多くの情報を得ることができるのかと驚きます。その過程で症例の病態について整理しながら考えることで臨床能力が身につくと感じます。
このようにPBLでは留学のために英語でディスカッションしたいという学生はもちろん、医学を丁寧に学びたい人にもぴったりなプログラムだと思います。
PBLは毎月の最終日曜日（日本の）に行う予定です！
はじめは英語でのディスカッションにハードルを感じる人も多いと思います。そんな方でも気軽に参加できるよう、見学のみでの参加も可能です！

Hello!
I am Yuki Hashimoto, a 6th year student at Asahikawa Medical University and a member of Team WADA student interns. This is the 5th time Clinical DOJO. Clinical DOJO is a PBL held between students from JABSOM and Team WADA student interns. I will be writing about this !
This time, 7 students from JABSOM joined us, including a new member, and we started the PBL with 15 students. Everyone looked nervous as they were meeting for the first time, but after introducing themselves one by one, the tension quickly dissipated. The case presentation was given by Shunya a, the director of this project. With the theme of dyspnea in a young male, we discussed possible diseases and what information about the patient we should ask. There were a lot of students and they all spoke positively, and we were able to discuss in a friendly atmosphere.
After the presentation of the case, we had a confirmation test using Quizizz! In the test, we were able to confirm the important points that we can learn from this case. We also got to know the JABSOM students better through questions about our Team WADA and Hawaii! Thanks to Shunya’s smooth moderating, ingenious presentation of cases and quizzes that encouraged the students to think actively, I was able to have a fun and very educational time!
There are two things that I feel make this PBL a very good project (there are more, but I will focus on them this time).One is the conversation with students from other universities and countries. Of course, discussions in English are interesting, but there is a more interesting point. We can learn other aspects of medicine through the conversations with students from other countries. This time, I learned that CT cannot be taken easily because of the cost of medical care, and that few people in the U.S. undergo routine medical checkups because medical costs are higher than in Japan. I think it is wonderful that we can learn about medicine in English and also interact with other cultures.
Secondly, I learned about the importance of interviewing in clinical reasoning. When many people share their opinions, I am amazed at how much information can be obtained from a medical interview alone. I feel that I can acquire clinical skills by thinking about the pathology of a case while organizing it in this process.
In this way, I think PBL is the perfect program for students who want to discuss in English in order to study abroad, as well as for those who want to learn about medicine carefully. I know that many people feel difficult to discuss in English at first, so even if you are one of those people, you are welcome to join us just to observe!
