Hello to all!

My name is Kana Fukuda and I am currently in Yamaguchi University Medschool in Year 4.

After a great interviewer Mr. Nishioka, I challenged the 8th interview which was,

“How to treat a patient with overweight and chief complaint of knee pain? and why?”

She was able to walk, speak and also loves to walk, though recently because of her knee pain, she was unable to start her exercise.

So what comes to your mind only with her chief complaints….?

Torn Cartilage? Sprained or strained knee ligaments? Arthritis?? or worst-case scenario…. Osteosarcoma??

In this interview, it was quite hard to tell the patient, whether I should tell the worst-case scenario, or just stick with the common symptoms such as sprained or Arthritis.

She eventually was diagnosed with OA (Osteoarthritis), but in order to diagnose this symptom, imaging is the key.

So what if a patient is a claustrophobia? Then what we could do..?

Perhaps she totally denied taking the imaging, then how could we be sure that the diagnosis is right?

Some ideas could be focused on breathing and covering patients’ eyes with towels, and some could be letting them listen to the music. Or if she or he is comfortable holding hands with partners or family members, then that might help.

Or if only we need the image of the lower part of the body, then we do not have to cover up all the patients with MRI and CT.

These rare cases could occur every day, so communication and ideas lead to an accurate diagnosis, which makes it happy for the patient and also for doctors.

Last not but least here are the questions from you.

Which of the following is NOT advised in patient with osteoarthritis?

A Avoid exercise

B Weight reduction

C NSAIDs

D Chondroitin supplement

E Chondoplasty

Yes, I made the mistake… Do not go directly to the pain-killers.. but let them exercise and eat healthy!
