必要な資格①：Must have completed, or be completing, a Royal College of Physicians & Surgeons of Canada (or equivalent) accredited residency program in Cardiac Surgery OR Vascular Surgery.
必要な資格②：Must be eligible for an Educational or Independent license to practice medicine by the College of Physicians & Surgeons of Ontario.
必要な資格③：Prospective Fellows must be proficient in spoken and written English and if language of medical school is not English, it may be necessary to pass the Test of English as a Foreign Language (TOEFL) and the Test of Spoken English (TSE) or the new TOEFL-lbt (http://web1.toefl.org/tstgprog01.cfm). Minimum scores are published under General Guidelines for University of Toronto Fellowships (see Additional Information).
必要な資格④：Preference will be given to candidates with experience with open and endovascular surgery in the thoracoabdominal aorta, prospects of a position at an academic centre, and a clear research focus.

Eligibility Requirementsとして羅列してくれていたのですべて拾いましたが、最低限の資格は①から③のようですね。③の英語の要件を詳しく見てみたかったのでリンクに飛んでみたのですが、cannot be foundでした。Speaking skillsが大事なようなのでSpeakingの最低限の点数と合計点、という感じでしょうか。
それと、⑤については前書きにたくさん書いたけど結局は最後に本音が出ちゃったという感じがして笑いました。
