久しぶりにRossi 先生の医療面接に参加しました。本日のお題は以下の通りでした。

Setting: General Medicine

Case:

The patient is a 78-year-old female known as a Type 2 diabetic. Currently, she is living at home alone because her husband passed away 3 years ago. Since then, she has developed dementia. The care staff come to see her 3 times a day. The care staff noticed that she has a grade 2 -3 ulcer on one big toe and leg. The wound seems to be infected. She is struggling to walk with a walking stick. Also, the staff noticed that she had frequently had a fall because the patient said that she could not see clearly. The staff are concerned that her vision might be a glaucoma.  The patient and her daughter have an appointment with you and will discuss a treatment plan for her condition with you today. She hasn’t seen her GP and Diabetic specialist last 3 years.

On admission, her BM is 21mmol/L , HbA1c is 9.1%, BMI : over 32

Medication: Metformin 500mg BD and Gliclazide 160 mg OD

Task:  Can you discuss with your Senior doctor about your treatment plan

私は事前に上司へのアセスメントを行いたいとお願いしていたのにもかかわらず、変に緊張をしてしまい、型を考えぬまま適当に話してしましました。わたしの下手なアセスメントに見かねたロッシ先生がSTAR method をご紹介してくださったので、復習も兼ねて皆さんに共有します。

STARメソッド
行動面接の一種で、限られた時間の中で、求職者の本質に迫ることのできるメソッドです。 「Situation ：状況」「Task：課題」「Action：行動」「Result:結果」の4つの頭文字、「S・T・A・R」を取ってSTARと呼ばれています。行動面接の一種ではありますが簡潔にわかりやすく伝える方法として非常に多くの場面で使用されています。

今回の症例であれば

S：

The 78-year-old female patient, a Type 2 diabetic living alone since her husband’s passing, has developed dementia and is visited by care staff thrice daily. She presents with a grade 2-3 ulcer on one big toe and leg, likely infected, struggles with mobility using a walking stick, and has experienced frequent falls possibly due to impaired vision, prompting concern for glaucoma. With no recent medical follow-ups, she and her daughter have an appointment to discuss a treatment plan for her complex conditions.

T:

The challenge we face is that the patient has dementia, which complicates our ability to assess her decision-making capacity. Additionally, her severe diabetes status indicates the need for immediate intervention to bring her glucose levels back to a normal range.

A:

I’ll order blood tests to assess her current condition. For the ulcer issue, I’ll arrange for drainage and a referral to a dermatologist. Regarding her eye problem, I’ll refer her to an ophthalmologist. And for the dementia, I’ll conduct a screening test and try to evaluate her decision-making capacity.

R:

と書くのが適切でしょうか？次回からはなるべくSTAR method を意識したアセスメントを作成できるようにします。

自分の反省点

・dementia の患者さんに対して、decision making capacity を調べようとしなかった点。

・患者さんに治療をしないことのリスクを伝えないまま、患者さんに選択させようとした点

・明らかに治療の経過から薬がincompliant もしくは治療の効果がないにもかかわらず、「ちゃんとやっている」

    という患者さんの言葉を信じて介入をしなかった点。

実際の臨床現場であれば、一日で首になってしまいそうですね笑

久しぶりに医療面接を行ってみましたが、できない部分が数多くあり、以前と成長していない（もしかすると後退している）ことに気が付きました。初心者の気持ちに立ち返って、積極的にこの勉強会に参加しようと思います。
